Daily rate includes the following services:
Private or Semi-private room
Three delicious meals daily planned by a licensed Dietitian
Nourishments and snacks
Housekeeping and laundry services
Social services
Activities
Restorative aide services
Routine 24 hour nursing care
Routine personal care/supplies
Incontinency care and supplies
Admission Preparation
When
it has been mutually agreed that the resident will be admitted to Foss Home & Village, we ask that you
provide the following information to make your admission process easier. On the day of admission, please plan
to meet with our admissions coordinator to complete the paperwork.
You will need to furnish:
All insurance cards (Medicare, Social Security, Supplemental, Major Medical and/or HMO). If applicable,
please furnish Medicaid card or award letter.
Copy of Long Term Care insurance policy and a copy of DSHS identification card and/or award letter when
applicable.
Legal documents: (Durable) Power of Attorney, Advance Directive, Living Will, Guardianship and any other
applicable documents.
Names, addresses and phone numbers of two contact persons in the event there is an emergency.
If admitting from home, a Physicians Order and History and Physical will need to be completed and signed by
the physician. Personal items, as space allows, help make the room more comfortable.
We recommend a minimum of seven changes of comfortable outfits and a week supply of underwear. Support-
ive shoes for therapy workouts when applicable. Each item will be labeled. All valuables should be left at home.
A $5000 payment towards the room is required upon admission.
Telephone service is the responsibility of the resident/family member.
