Site/Facility:
Hanford/All
Event: On or about October 24
th
of 2007 suspect rigging hardware was found in a weather enclosure/fabric structure
at the 218-E-12B low level burial grounds, this hardware was being used to secure the structure during high winds
and snow loads. The items found at this time were 148 5/8

and 75 7/8 shackles, these shackles were missing the
manufactures name or trade mark as required by ASME B30.26 and are marked China. Further investigation
found additional suspect items such as cable clamps, turnbuckles, and aircraft cable. The cable clamps and
turnbuckles were also missing markings as required by ASME B30.26. The aircraft cable spool was missing the
manufactures name or trade mark, wire classification, test reports, and the wire construction didnt match what was
stated on the spool. A number of these suspect items (shackles, turnbuckles, cable clamps and cable) were selected
for testing and failed well below the expected load requirements.

On or about December 5
th
of 2007 during receipt inspection of a weather enclosure/fabric structure at 105 KE/100K
suspect turnbuckles were found, these turnbuckles have various length cables attached and are also in question. In
both cases nonconformance reports and occurrence report were written.

On December 12
th
of 2007 inspection of weather enclosures/fabric structures at the Rattlesnake barricade, Plutonium
Finishing Plant, and TRU retrieval trench 4B found suspect turnbuckles thought out the structures.

Important Points:
Regardless if the items are used in a rigging application or not it is important to view these
as being substandard as it pertains to strength and load calculations based on use of rigging hardware in this
application should be questioned.


Actions:
The Fluor Hanford S/CI Interpretative Authority has been directed to request all Hanford Site S/CI
Point Of Contacts, in their respective areas, to inspect these structures for the use of S/CI listed in this alert and to
identify as required by HNF-PRO-301 or the controlling companies S/CI program.

S/CI indicators:
Shackles shall have name of manufactures name or their trade mark (China is not a manufactures name or
trade mark), working load limit, and shackle size.
Turnbuckles shall have manufactures name or their trade mark (China is not a manufactures name or trade
mark), working load limit or size, and if it is an alloy material then the grade shall be listed.
Cable clamps shall have cable size, and manufactures name or their trade mark (China is not a
manufactures name or trade mark).
Wire rope (cable) shall have listed on the spool manufactures name or their trade mark (China is not a
manufactures name or trade mark), size and construction, classification and is normally ordered with test
reports.
