Notes and recommendations from Town Hall Meeting June 22

In junction with the annual CMSA meeting, a Town Hall Meeting was hosted to take an initial
look at the first draft of the Case Load Matrix developed by the CMSA/NASW Work Group this
spring. Panelists included:

Mary Ellen Gervais, RN, MS, CCM

Cheri Lattimer, RN, BSN

A brief overview was presented describing the current draft of the Case Load Chart. Throughout
the Town Hall Meeting, several dozen questions and comments were made by the audience. Here
are some highlights. Several comments were made addressing:


The scope of the case load matrix:
o
Add under Type of Medical Management Services Transition of Care
o
Also add administrative task such as staff meetings, committee projects, QI
activities, reporting responsibilities
o
Another request from the military to add Trauma Assessment
o
Work Comp include LTD, RTW, % of Disability
o
Under Initial Elements Impacting Case Load, change Social work to
behavioral health to reflect the variety of professionals (such as psych nurses)
who work in behavioral health. The change could also reflect that social workers
(and a variety of professionals) work in health/medical settings--already a
category under Market Segment--as well as in behavioral health.


The impact of the case management process
o
Need to enhance the outcome and productivity expectations
o
Expand on level of care/acuity thresholds and patient volumes
o
Add staff satisfaction to Outcomes


The application in other settings:
o
Address the additional needs of the Military
o
Expand the analysis to address hospital settings covering the full range of
services from the initial assessment to discharge planning
o
Define where the Community Health Clinic fits in


Methodological and definitional issues:
o
Need to create a definitional section to explain key terms
o
Add and adequacy to caregiver willingness under Psychosocial Factors
Family
o
Under Psychosocial FactorsEnvironment, change Residencehome,
facility to read level of care.
o
Can we enter time units related to tasks
o
Documentation requirements should be considered (e.g., whether paper-based or
computer-based and to what extent)

General issues:
o
Opportunities are great
o
This is great timing

Many challenges but good start

Offers to work on project from many of the attendees
o
Stated the work would be up for public comment by the end of July
o
White paper in good next step
