Contract Term:

Contract Amount: $348,018.48
CONTRACT
This is a general services Contract, (Contract), entered between
County, (County),
a body politic and corporate of the State of Arizona and
SER Jobs for Progress of Southern
Arizona, Inc (Contractor);
WHEREAS, County requires the services of a Contractor to provide occupational skills
training for youth selected and referred by the County;
WHEREAS, pursuant to Contractors proposal submitted on May 10,2002 in response to
the Workforce Investment
Councils Request for Qualified Vendors for Youth
Occupational Skills Training
Contractors expertise was
determined to be advantageous to County; and
WHEREAS, Contractor has collaborated with the school districts to provide occupational
skills training and is willing to provide such services on an as-needed contract subject to
notice to proceed;
NOW, THEREFORE, the parties agree to enter in a contract, consisting of this document
and any attachments, which are fully incorporated by reference, as follows:
ARTICLE
I
This Contract shall commence on the first day of June 2002, and shall terminate on the
thirtieth day of June 2003
sooner terminated. Services must be authorized by a
Notice to Proceed issued by the Director of Community Services. The County may, at any
time, modify Notices to Proceed and the extent or amount of services. Contractor has no
exclusive rights as a result of this Contract, nor is there any guarantee to the amount of

services requested. The County shall have the option to extend this Contract for up to 12
additional months.
ARTICLE
PAYMENT
A. This is a unit cost contract. Contractor shall be paid for services requested by the
County in accordance to the schedule in Attachment A.
1. All work statements shall be entitled to payment by the following schedule:
The first 25% of the total cost per student shall be paid after the student
has attended 3 days
of
the program.
ii.
The second 25% shall be paid after 33 days of attendance
iii.
The third 25% shall be paid after 66 days of attendance
iv. The final
25%
of the total cost shall be paid when the student completes
the program and has received a certificate of completion

Contractor shall receive up to $3,324.07 per youth for a
total not to exceed $79,777.68

Contractor shall receive up to $5,562.68 per youth for
a
total not to exceed $1 03,544.32
4. Work Statement
Contractor shall receive up to $5,614.02 per youth for a
total not to exceed $1 34,736.48
Payment for the term
of this Contract
shall
not exceed $348,018.48
C.
Request for payment for services under this Contract must be certified on invoices,
signed by an authorized representative of Contractor, and supported by
documentation, including attendance records and completion certifications. County
representatives will verify these documents. An approved County form of invoice is in
Attachment
All invoices shall be submitted to Community Services Department, 32

D.
Changes between budget line items totaling less than 15% of any line item may be
granted by the County Director of Community Services following Contractors
submission of a written request provided that any proposed increase is offset by a
decrease of equal value to the remaining line items.
Granting of a qualifying change request is within the discretion
of

Work Statement No. 1
Title: Junior-Senior Familiarization Program for Health Careers in Certified Nursing
Assistant
(CAN) and Patient Care Technician (PCT).
Type: Unit Cost
A.
Funding
Funding shall be from a variety of sources that may include Youth Opportunity,
Workforce Development Act, and Pima County General fund. The funding source will
be
based on the Youths intake information established by the Youth Service
Specialist who
will
refer the youth.
B. Program Overview
Contractor shall provide a year round program to train youth and prepare them to be
employed as a Certified Nursing Assistant and/or a Patient Care Technician at
Sunnyside High School.
C.
Program goals
Prepare high school youth for a career in the health care industry.
D. Program activities
Contractor shall:
1.
Provide clinical experience and classroom training to prepare students to enter
the workforce as a CNA and/or PCT.
2.
Locate program at Sunnyside High School
3.
Provide summer and school year training to referred youth
4.
Provide

total hours of both clinical experience and classroom training
5.
Pay students stipends for the students clinical hours, and Patient Care
Technician time up to

hours
6.
Ensure students are provided the supplies they need to participate

Hire and supervise school year clinical instructor and summer instructor
8. Assist youth to register for Pima College Classes and pay required student
fees
9.
Register and pay students CNA licensure test fees and certificate fees

Obtain a signed work site agreement with work site.
County shall:
Recruit youth, determine their eligibility for available funding and provide the
referred youth written verification of referral indicating funding source, program
in which the youth
is
referred and the County One Stop Case Manager who
made the referral.

Participates must be

High School Junior, pass a drug test, have at least a
grade
reading and math level, be at least 16 years of age and be referred through the One
Stop Case Manager.

Contractor shall serve up to 24 youth.
H. Outcomes
Completion

90%
of the referred youth shall complete 90% of the available hours and
receive a certificate of completion.

of the youth who complete number 1 above shall achieve their CNA
certificate

of the youth who complete number 2 above shall achieve their
PCT
